Series is an artificial creation by the Archivist. Items were found in the Archives/Museum loose with other unrelated materials. Items were created by the School of Nursing for student nurses to fill in. Items were subsequently donated to the Alumni Association for the Archives from the School of Nursing at some point. Items were not accessioned at time of receipt and no other provenance is known.

Scope and content

Series contains various types of blank form used by the WGH and HSC Schools of Nursing. They include the Student Sick Call, a form to be filled in when a student called in sick whilst at Health Sciences Centre School of Nursing and a form which was used by the students for documentation of their clinical objectives. Once the objectives were met the form was signed by instructor ward nurse, or the senior student supervising the procedure. Information for the clinical objectives form came from Anne Crossin and was written on one of the forms. Also included are two envelopes, historical record of diplomas, student permanent record and a blank clinical chart.
